By submitting this application, you authorize $12 to be deducted from your payroll bi-weekly. Once we receive and process your application, we will contact you to confirm your membership.

Please note that if you require COTAPSA assistance within the first six months on joining, you may be required to pay back-dues.  This is required to encourage non-union employees to join before they have an issue and not only at the time when assistance is required.
